YOUR FINANCIAL RESPONSIBILITY
If you have medical insurance, Associated Obstetricians and Gynecologists, Ltd. will gladly file insurance claims for you. We make our best effort to ensure that the information sent to the insurance company is accurate and clearly describes the services you received. Many of the services provided in this office are covered and paid by your insurance company. However, in cases where the service has not been paid by your insurance within 60 days, you will be personally responsible for the bill.
Deductibles and payment for non-covered services are due at the time of services. If your insurance company does not cover the services you desire (such as services provided by a Certified Nurse Midwife) or if you do not have insurance coverage, Associated Obstetricians and Gynecologists, Ltd. will provide you with the opportunity to meet with a Patient Financial Counselor. A financial agreement form will be completed which will include the cost of the surgery, any deductible due, an estimate of the expected insurance payment at out-of-network rates as applicable, and an estimate of the amount you will need to pay for the service. Financial arrangements can be discussed and a payment plan arranged if necessary.
ANNUAL EXAMINATIONS
As a commitment to your health, we recommend that every patient have an annual examination that allows us to evaluate your overall health picture, and to make sure you are not developing any unexpected problems. Medicare authorizes an annual exam once every 24 months.
During this visit we will update all of your known conditions, as well as look for any new problems. Unless there is some major new finding during this examination, we must submit the service to your insurance company as an annual examination which may not be paid by your insurance company.
Along with the examination, the doctor might suggest that some screening tests be performed. These services may also be considered non-covered by your insurance company and you will be expected to pay for them yourself.
We will be happy to work with you on a payment plan for non-covered services, but these arrangements must be made in advance. If you have questions, please feel free to contact our Billing Office at (414) 328-8598.
INSURANCE FILING AND THE LAW
Federal law requires that we submit every claim to an insurance company accurately, reporting the exact services performed and the exact reason for performing them. We cannot change information on an insurance claim just so the claim will be paid.
Our practice is committed to the law and will submit all claims to all insurance companies in this manner.
LABORATORY SERVICES
If your insurance plan requires that you use a specific laboratory, please notify us so that specimens are routed to the appropriate lab. Services provided by outside laboratories will be billed directly to you.
